Mountain View Pediatrics will be conducting a monthly drawing for 4 free movie tickets to Marquee Cinemas in Morganton. To enter the drawing, complete the short patient satisfaction survey below. Please add you name, email address, and/or phone number at the end of the survey.

(Surveys submitted during the same calendar month will be eligible for the drawing. Limit one survey entry per month per family. The drawing will occur the last day of each month. Winners will be contacted by phone or email.)
